Sw1mone Swimming Lesson Agreement

Thank you for choosing Sw1mone for your swimming lessons.

Before you can start your swimming lessons with Sw1mone, you must agree to the terms and conditions, that are stated within the 'Swimming Agreement' form below, and sign the form. The form is located on this webpage, and is now fully online, with no more paper forms to sign!

The form will ask you to agree to the terms and conditions, provide emergency contact details, as well as any important medical information regarding the parent, or legal guardian, and the swimmer that we should be aware of.

For your records, you will be asked for an email address within the form, where a copy of your completed agreement will be sent.

We have tried to make the form as easy as possible to fill out, and it should only take you about 5 minutes to complete.
